2 ways (in my mind) to accomplish this.
1) use the cropping tool the keep the 16X9 ratio
if you are zooming you could use keyframing to maintain the 16X9
format

I've had this happen and it has usually been due to a corrupt (rendered) file.
What I do to find the corrupt file is load only 1/2 of the renders on the time line... then try to render it out.. if...
