Well..

On 9.5 era

and On 9.5.1 wich have BULGE with map option

Any ideas or tutorial about doing a realistic neck rig like this?

Go through the Messiah threads on CGTalk and you'll find a discussion with Taron about a very simple rig that he used for that. The principles will be the same.

In Lightwave, you could have a normal displacement image map, where it's coordinates are controlled by an object reference. The object is a null which is parented to the neck bones.

I wonder if a bump image map would also suffice, for maybe a cheaper, quicker look.
