Tally Light system for VT

I'm looking for a Tally light system for the VT. I have the pin outs, but what lights do i need? i would prefer to purchase an all in one system with 4 lights. any suggestions on who makes one of these or where to purchase them.

I wouldn't mind building a system if someone could point me to some plans or whatnot.
